You are appointing other people to make decisions on your behalf. You are the 'the donor'.

Most people want both forms, some want to cover only limited situations.
The person/people you choose to make decisions for you are called your attorney(s). The should be people you trust and know well. You need at least one attorney, but you can have more.
Severally means individually. Be careful if you want all the attorneys to make decisions together, as if one dies, the others become unable to act. You should list the decisions you want your attorneys to make jointly, or severally, if relevant.
If one of your original attorneys is unable or unwilling to act, or is no longer legally your husband, wife or civil partner.
Most people choose the first option because it is most practical and note, your attorneys can only operate with your consent whilst you have mental capacity. Be careful with the second option, because it makes your LPA a lot less useful, and your attorneys might be asked to prove you don't have capacity each time they use the form.
You can let people know you're going to register your LPA. They can raise any concerns they have, for example, if they think there was any pressure or fraud in making it. You can't put your attorneys here.
You can tell your attorneys how you'd prefer them to make decisions, or give them specific instructions which they must follow. Most people leave this section blank.
